Checklist:    SALE/LEASE/ OR TRANSFER

THE APPLICATION PROCESS TAKES 15 WORKING DAYS

***ALL ASSESSMENTS MUST BE PAID UP TO DATE FOR APPROVAL***

Applications without original signatures or missing any of the documents below will not be accepted NO EXCEPTIONS!!!

1. Complete application with ORIGINAL SIGNATURE(S) leaving no empty spaces
2. Complete Lease Addendum
3. Copy of Driver's license/picture ID for each applicant/occupant over the age of 18
4. Copy of Vehicle registration(s). If not registered under applicant name, letter of explanation.
5. PROVIDE Three (3) references for each applicant over the age of 18.
   (must provide name, address, & telephone of the person giving the reference) They must be unrelated and must be known for at least 1 year.
6. Lease Agreement or Purchase Contract (Fully Executed)/plus Addendum if Applicable
7. Low credit score and criminal background may be grounds for Denial of Application.
8. Money order or cashier check (NO PERSONAL CHECKS OR CASH) payable to "VILLE D'OR CONDOMINIUM ASSOCIATION" for the application fee: $100.00 per adult over 18 years of age, $150 International applicants, $300 for a Corporation or LLC.

Service/Emotional Support Documentation (Required by HUD) Section 760.27(2)(b), Florida Statutes provides as follows:

 (b) If a person’s disability is not readily apparent, request reliable information that reasonably supports that the person has a disability. Supporting information need to include:

1. A determination of disability from any federal, state, or local government agency.

2. Receipt of disability benefits or services from any federal, state, or local government agency.

3. Proof of eligibility for housing assistance or a housing voucher received because of a disability.

4. Information from a health care practitioner, as defined in s. 456.001; a telehealth provider, as defined in s. 456.47; or any other similarly licensed or certified practitioner or provider in good standing with his or her profession’s regulatory body in another state but only if such out-of-state practitioner has provided in-person care or services to the tenant on at least one occasion. Such information is reliable if the practitioner or provider has personal knowledge of the person’s disability and is acting within the scope of his or her practice to provide the supporting information.

5. Information from any other source that the housing provider reasonably determines to be reliable in accordance with the federal Fair Housing Act and s. 504 of the Rehabilitation Act of 1973.

Once the application has been processed and the background completed, the application will be submitted to the Board of Directors for their final decision and interview if necessary.

NO RESIDENT CAN MOVE IN WITHOUT BOARD APPROVAL! IF YOU MOVE IN WITHOUT BOARD APPROVAL YOU WILL BE VACATED!
